Incidents are planned as a dedicated Fear Index signal. They are not live in the composite yet because the site needs a reliable, authorized feed before incident counts affect the headline score.

Until the feed is connected, incidents are displayed as unavailable and their weight is removed from the live index.
What should count
- Publicly documented AI failures, harms, misuse events or near misses.
- Events with enough detail to classify date, system, sector and severity.
- Incidents from a repeatable source rather than one-off manual collection.
Planned scoring approach
The incident signal should eventually combine recent incident volume, severity and recency. A single major incident should matter, but a cluster of moderate incidents should also raise the score.
